Mandatory elements
This page explains, in English, what this notarized Bulgarian document — съгласие и образец от подпис (consent and specimen signature) — must contain. It combines two functions: an express consent to accept the position, and a specimen signature against which the manager's later signatures on the company's documents can be verified:

Why notarization is required
The Commercial Register has no other way to verify the new manager's identity than through a notary. The notarization certifies that the signature was made in person by the named individual, and serves as an official reference against which the manager's signatures on invoices, contracts, and other company documents can later be compared — including by banks and counterparties in case of doubt about authenticity.

Is notarization mandatory?

Yes, the manager's signature on the consent and specimen signature document must be notarized under Art. 141(3) of the Commerce Act.
Does the manager need to appear in person before the notary?

Yes, in person and with an identity document — the notary certifies that the signature was made by the named person.
Is the document filed with every subsequent application?

No, only on the manager's first registration with the Commercial Register or when their signature changes.
